ADDENDUM NO. 1

 

This addendum has been issued by the Town of Penetanguishene Public Works Department to clarify specific items of the request for proposal (RFP) document.

 

The following sections are being clarified within the RFP document:

 

(Section 5.1, Item 9. –AODA Training Requirements)

Sealed Submissions labelled with the template provided must be received by the Town of Penetanguishene, Corporate Services Office, 10 Robert Street West, Penetanguishene, Ontario PO Box 5009 L9M 2G2 on or before

11:00 a.m. EST Wednesday January 17th, 2018

 

3.18 SCHEDULE FOR RESPONDENT SELECTION

 

            Proposal Closing                        January 17th, 2018

            Notification of Selection              March 15th, 2018

 

 

 

 

Section 5.1, Item 9. – Accessibility training as required by AODA of all employees, agents, volunteers and subcontractors hired by us in completion of our work has been completed.

Please indicate YES/NO: ________________ If YES, attach a copy of certificates. If NO, the Vendor Form provided by the Town should be completed prior to commencing the work.

 

If the awarded vendor has not completed AODA required training, the Town will issue a training document to the awarded vendor which meets the intent of the standard.  If the vendor has completed AODA training, then certificates can be provided.  In other words, the Town will assist the awarded vendor to achieve compliance with the AODA Standards if the vendor has not previously completed training.

4. Concrete Compressive Strength Testing (CCIL Certified Lab) Concrete testing for the sidewalk, curb and gutter installation will include slump and air entrainment testing for each load delivered for acceptance. The sampling of concrete for compressive strength testing will be in accordance with the CSA standards for concrete testing. Each bidder may clarify the number of sets and cylinders to accompany their bid.
